Rivaroxaban falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Ketogenic Diet interacts with.

Although there is some concern that the ketogenic diet might have additive effects when used with anticoagulant and antiplatelet drugs, most clinical research does not show this effect.
One small observational study found that the ketogenic diet was associated with prolonged bleeding times in 16 of 51 (31%) children with epilepsy who were also on chronic anticonvulsant therapy. However, a larger observational study of 162 children with epilepsy taking chronic anticonvulsants has found that following the ketogenic diet does not increase the risk of bleeding, even in the 25 children undergoing surgery. Research in adults also has not reported changes in bleeding time.

Here's the short version: if you take Xarelto (rivaroxaban) and follow a ketogenic diet, the worry on file is that the two together could thin your blood a little more than expected. The thinking is that very low-carb eating might add to the blood-thinning effect, raising the theoretical chance of easy bruising, nosebleeds, or bleeding that's slow to stop.

The good news is that the better evidence doesn't really back this up. Most studies, including a larger one in children and research in adults, found no real change in bleeding when keto was added on top of medications. So this is rated minor and unlikely. Still, since rivaroxaban already affects bleeding, mention any new diet plan to your pharmacist or doctor, and watch for unusual bruising or bleeding.

What Is Rivaroxaban Used For?

Brand name: Xarelto

Rivaroxaban tablets are used to reduce the risk of stroke in adults with nonvalvular atrial fibrillation (an irregular heart rhythm), treat deep vein thrombosis (DVT, a blood clot in a vein) and pulmonary embolism (PE, a blood clot in the lungs), and lower the chances of these clots coming back. They also help prevent clots after hip or knee replacement surgery, protect hospitalized patients at risk for clots, and — in combination with aspirin — reduce the risk of major cardiovascular events in adults with coronary artery disease (CAD) or peripheral artery disease (PAD).

Rivaroxaban tablets are also approved for treating VTE (venous thromboembolism, blood clots in veins) and preventing recurrence in pediatric patients from birth to under 18 years, and for preventing clots in children aged 2 and older with congenital heart disease after the Fontan procedure. Rivaroxaban for oral suspension (available as Rivaroxaban Granule and Xarelto oral suspension) is specifically indicated for these two pediatric uses.
