Theoretically, following the ketogenic diet while using antidiabetes drugs might increase the risk of hypoglycemia.
The ketogenic diet involves a very low intake of carbohydrates, which can result in decreased blood glucose levels in patients taking antidiabetes drugs. In some patients, the need for antidiabetes drugs is reduced. Hypoglycemia has also occurred in some individuals without diabetes, although this is uncommon.

Here's the thing to keep in mind: repaglinide works by nudging your pancreas to release more insulin, which lowers your blood sugar. A ketogenic diet is very low in carbs, so it naturally pulls your blood sugar down on its own. Put the two together and you can stack one sugar-lowering effect on top of another, which raises your chance of going too low (hypoglycemia).

If that happens, you might feel shaky, sweaty, dizzy, hungry, confused, or get a fast heartbeat. This is mostly a theoretical concern based on limited evidence, so don't panic, but it's worth watching. Please don't stop or change your repaglinide on your own. Talk with your doctor or pharmacist first, check your blood sugar more often if you go keto, and they can adjust your dose if needed.

Repaglinide tablets are used to help control blood sugar in adults with type 2 diabetes, together with diet and exercise. It is not a substitute for healthy habits — it works best as part of a full diabetes management plan.

Repaglinide is not for type 1 diabetes or for treating diabetic ketoacidosis (a dangerous condition where the blood becomes too acidic due to lack of insulin).
