Why this applies to Brexanolone

Brexanolone falls under CNS DEPRESSANTS — the group Kratom interacts with.

Theoretically, concomitant use might increase the risk for adverse CNS depressant effects.
Kratom contains mitragynine, a mu-receptor agonist that may cause respiratory depression. Although results from animal research suggest that mitragynine causes less respiratory depression than the CNS depressant codeine, fatalities have been reported for patients who ingested a combination of kratom and O-desmethyltramadol, another mu-receptor agonist. Additionally, observational research suggests that adverse effects such as drowsiness and coma are more likely to occur in individuals taking kratom in combination with agents such as opioids and sedatives. Theoretically, ingesting kratom along with other CNS depressants can increase the risk of adverse effects, including drowsiness, coma, and/or severe or fatal respiratory depression.

Here's the thing to know about Zulresso (brexanolone): it's already a strong sedating medicine. It's given as a slow IV drip in a hospital for postpartum depression, and on its own it can make you very drowsy, dizzy, or even cause sudden sleepiness or fainting. That's why nurses watch you closely the whole time.

Kratom acts on the same opioid receptors as pain medicines and can add its own calming, slowing effect on the body and breathing. Putting the two together could stack the sedation, meaning deeper drowsiness, loss of alertness, and in the worst case slowed breathing. This concern is based on how the drugs work rather than hard studies, but with a medicine this sedating I wouldn't take chances. Please tell your care team about any kratom use before treatment.
