Why Are There 2 Interactions?
Kratom interacts with whole categories of medication. Modafinil falls into 2 of those categories, so it can interact with Kratom in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kratom interacts with.
In vitro research shows that kratom extract inhibits CYP3A4 enzyme activity. A pharmacokinetic study in adults shows that taking kratom tea 2 grams modestly increases the plasma concentration time-curve (AUC) and maximum concentration (Cmax) of CYP3A4 probe midazolam by 40-50% and its CYP3A4-mediated metabolites 18-27%. However, a lack of change in the half-life of midazolam and a simulation of the kratom-midazolam interaction suggest that kratom inhibits CYP3A4 enzymes in the small intestine but not the liver. Additionally, there is one case report of a 27-year-old male who died from neuroleptic malignant-like symptoms after concomitant use of kratom and quetiapine, a CYP3A4 substrate. Although it did not appear that the patient had consumed large quantities of quetiapine, postmortem plasma levels were in the lethal range, indicating possible inhibition of CYP3A4 by kratom. In another case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It is hypothesized that CYP3A4 inhibition by kratom reduced metabolism of desvenlafaxine, trazodone, and ziprasidone and consequently increased systemic exposure to serotonin.

Here's the basic idea: your body uses an enzyme called CYP3A4 to help clear modafinil, and kratom appears to slow that enzyme down a bit (mostly in the gut). Modafinil is the active drug itself, not a prodrug, so blocking its breakdown could let a little more of it build up in your system. In plain terms, that might mean a stronger or longer effect, with side effects like jitteriness, headache, a racing or pounding heart, trouble sleeping, anxiety, or nausea showing up more easily.
I want to be honest about the strength here: this is mostly theoretical, based on lab work and one human study using a different drug. So it's a maybe, not a sure thing. Still, since kratom carries real risks of its own, please check with your pharmacist or doctor before combining the two.

MODAFINIL (Provigil)
2 of 2A 43-year-old male patient experienced generalized tonic-clonic seizures after adding modafinil 100 mg to his chronic ingestion of kratom tea four times daily to further improve alertness and as a self-treatment for opioid withdrawal. The exact mechanism by which this interaction may occur is unknown.

Here's the gist for combining kratom with modafinil (Provigil): the main worry is a possible increased risk of seizures. Both substances are stimulating to the brain, and the concern is that stacking them may lower the threshold for a seizure. This isn't about kratom changing how modafinil is broken down. It's more about two activating substances pushing the brain in the same direction.
I want to be honest about how strong this evidence is: it's quite thin. It comes mainly from a single case report of a man who had a seizure after adding modafinil to heavy daily kratom use, and the exact reason isn't known. So it's more theoretical than proven. Still, a seizure is serious, so please don't combine these on your own. Talk with your pharmacist or doctor first, especially given kratom's other real risks.

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).
In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
