Trandolapril, Verapamil falls under ANTIHYPERTENSIVE DRUGS — the group L-arginine interacts with.

Theoretically, concomitant use of L-arginine and antihypertensive drugs may increase the risk of hypotension.
L-arginine increases nitric oxide, which causes vasodilation. Clinical evidence shows that L-arginine can reduce blood pressure in some individuals with hypertension. Furthermore, combining L-arginine with some antihypertensive drugs seems to have additive vasodilating and blood pressure-lowering effects.

Tarka is a combo pill: trandolapril (an ACE inhibitor) plus verapamil (a calcium channel blocker). Both are designed to relax your blood vessels and bring your blood pressure down. L-arginine works on a similar path, since your body turns it into nitric oxide, which also widens blood vessels. So when you stack them, the blood pressure lowering can add up.

This is a theoretical concern based on how these work, not a guaranteed problem, but it's reasonable to watch for. If your pressure drops too low, you might feel dizzy, lightheaded, tired, or flushed, especially when standing up. Please don't stop or change either one on your own. Just check with your pharmacist or doctor before adding L-arginine, and consider monitoring your blood pressure at home.

Trandolapril, Verapamil also falls under ACE INHIBITORS (ACEIs).

Theoretically, concomitant use of L-arginine and ACE inhibitors may increase the risk for hypotension and hyperkalemia.
Combining L-arginine with some antihypertensive drugs, especially ACE inhibitors, seems to have additive vasodilating and blood pressure-lowering effects. Furthermore, ACE inhibitors can increase potassium levels. Use of L-arginine has been associated with hyperkalemia in some patients. Theoretically, concomitant use of ACE inhibitors with L-arginine may increases the risk of hyperkalemia.

Tarka combines two blood pressure medicines: trandolapril (an ACE inhibitor) and verapamil (a calcium channel blocker). Both already relax and widen your blood vessels to lower pressure. L-arginine works in a similar direction by boosting nitric oxide, which also widens vessels. So stacking them could add up and push your blood pressure too low. Watch for dizziness, lightheadedness, feeling faint when you stand, or unusual tiredness.

There's a second piece: ACE inhibitors like trandolapril can raise potassium, and L-arginine has been tied to higher potassium in some people too. Too much potassium can affect your heart rhythm. Keep in mind this concern is mostly theoretical, based on how these work rather than strong studies. Please check with your pharmacist or doctor before adding L-arginine, and don't change your Tarka on your own.
