Here is the interaction we found between these two, with the evidence behind it.
ANTIBIOTIC DRUGS
Why this applies to Pivmecillinam
Pivmecillinam falls under ANTIBIOTIC DRUGS — the group Lactobacillus Helveticus interacts with.
Lactobacillus helveticus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. helveticus preparations by at least two hours.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep in mind: Pivmecillinam is an antibiotic, and antibiotics don't tell the difference between the bacteria making you sick and the friendly bacteria in a probiotic. So when you swallow Lactobacillus helveticus right alongside your pivmecillinam, the antibiotic can kill off a good chunk of those live probiotic organisms before they ever get to work. This doesn't make your pivmecillinam any weaker or more dangerous, and it won't change how well it treats your infection. It mostly just wastes the probiotic.
This is a theoretical concern based on how antibiotics behave, not proven harm. A simple fix is spacing them out by at least two hours. If you're unsure about timing, your pharmacist can help you map out a schedule.

The most serious there right now: Activated Charcoal (major), Bacillus Coagulans (moderate), Bacillus Subtilis (moderate), Bifidobacterium Animalis Subsp. Lactis (moderate) and Bifidobacterium Bifidum (moderate).
By severity: 1 major · 33 moderate · 10 minor.
From our pharmacist’s overview on the Pivmecillinam page
“If you take supplements alongside pivmecillinam (Selexid), an antibiotic used mainly for urinary tract infections, a few pairings deserve genuine caution. Activated charcoal carries the only major rating among the 44 ingredients on file, because it soaks up medicines in the gut and can sharply cut how much antibiotic your body absorbs.”

About Lactobacillus Helveticus
What is Lactobacillus Helveticus?
- Mood and stress support
- Digestive and gut health
- Sleep support
- General immune support
- Blood pressure support
Lactobacillus helveticus is a lactic acid bacterium used as a probiotic, often combined with other strains in mood, gut, and general wellness products. Early research is interesting—especially for stress and mood and for fermented-milk peptides and blood pressure—but evidence is still limited and not conclusive. It is generally well tolerated in healthy people, but talk to your pharmacist or doctor before using it, especially if you have a weakened immune system.
