Treprostinil (IV) falls under Anticoagulant/Antiplatelet Drugs — the group Mullein interacts with.

Lab tests on extracts from different parts of common and dense-flowered mullein plants show that they do have some blood-thinning effects, as they increase the time it takes for blood to clot. This is likely due to at least one of the known constituents of muellein, triterpenoid saponins. However, the effects appear to be mild and results have been inconsistent. Extracts from different plant parts have been show to have varying anticoagulant strengths, with water-based ones being more potent than alcohol-based extracts.

Here's the short version: treprostinil (Remodulin) isn't a blood thinner itself, but it does relax blood vessels and can make your platelets a little less sticky, which can mildly raise bleeding risk on its own. Mullein has shown very weak, on-and-off blood-thinning activity in lab dish studies only, mostly from its saponins. So in theory, layering the two could nudge your bleeding risk up slightly.

In real life, I wouldn't lose sleep over a cup of mullein tea. The evidence here is mild and inconsistent, not from people taking it with this medicine. Still, since treprostinil is given IV for pulmonary hypertension and is serious therapy, it's worth a quick mention to your doctor or pharmacist, and watch for easy bruising or bleeding that won't stop.

What Is Treprostinil Injection Used For?

Brand name: Remodulin

Treprostinil injection is used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — high blood pressure in the arteries that supply the lungs. It works to reduce the symptoms that make exercise difficult. Clinical studies included patients with moderate-to-severe PAH symptoms (NYHA Class II–IV) due to idiopathic or heritable PAH, PAH related to congenital heart defects, and PAH related to connective tissue diseases.

It is also used for PAH patients who need to transition off epoprostenol — a related medication — to slow the worsening of their condition during that switch. The risks and benefits of the transition should be carefully discussed with a specialist before starting.
