Sotalol Hydrochloride falls under BETA-BLOCKERS — the group Octacosanol interacts with.

Theoretically, concomitant use of octacosanol with beta-blockers might result in additive hypotensive effects.
Octacosanol is the main component of policosanol. Clinical research shows that policosanol 5 mg daily can have additive blood pressure-lowering effects in patients with hypertension who are taking beta-blockers. Also, animal research shows that policosanol can increase the hypotensive effects of propranolol. It is not clear if these effects occur with the use of octacosanol.

Here's the gist: sotalol already works to lower your blood pressure and slow your heart rate, and octacosanol (the main piece of policosanol) may nudge blood pressure down a little further. Put them together and those effects could stack up. In practice that might show up as dizziness, lightheadedness when you stand, unusual tiredness, or a heart rate that feels slower than normal.

I'll be honest about the evidence. The clearest data come from policosanol, not octacosanol itself, so it isn't certain this happens with octacosanol alone. Sotalol is a drug we watch closely because it also affects heart rhythm, so I'd rather you not guess. Before adding this supplement, check in with your pharmacist or doctor and keep an eye on your blood pressure.
