Fiorinal with Codeine already carries two ingredients that calm the nervous system: butalbital (a barbiturate) and codeine (an opioid). Both can make you drowsy, foggy, or slow your breathing, especially at higher doses or when combined with other sedating things. The concern here is simply additive: some animal studies hint that the berberine in phellodendron may have its own mild sedative effect, so stacking it on top could deepen the sleepiness or sluggishness you already feel from this medicine.
I want to be honest that this is theoretical, based on lab and animal work, not human reports. Still, if you notice extra grogginess, dizziness, or trouble concentrating, that is worth mentioning. Please check with your pharmacist or doctor before pairing the two rather than adjusting anything yourself.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Here's the wrinkle with Fiorinal with Codeine: the codeine part is a prodrug. Your body needs an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Phellodendron contains berberine, which in lab and early human research can block that same enzyme.
So if berberine slows CYP2D6, less codeine gets converted to morphine, and you may simply get less pain relief than expected. The other ingredients (aspirin, butalbital, caffeine) aren't the main concern here. Keep in mind this is a theoretical interaction based on how the body works, not strong proof. Still, before pairing them, check with your pharmacist or doctor so your headache relief isn't quietly working against you.

Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist with Fiorinal with Codeine. The berberine in phellodendron can mildly slow down a liver enzyme called CYP3A4, and a few ingredients in this combo pill (notably butalbital and codeine) lean on that pathway. In theory, blocking it could let a little more of those ingredients linger in your system. That might mean more drowsiness, dizziness, or feeling groggy than usual.
One twist worth knowing: codeine is a prodrug, meaning your body has to convert it into morphine to relieve pain. That step mostly runs through a different enzyme (CYP2D6), so the painkilling effect itself is less likely to change much here. Honestly, this whole concern is theoretical and based on lab work, not real patient reports, so I wouldn't panic. Just give your pharmacist a heads-up before combining them.

The piece of Fiorinal with Codeine that matters here is the aspirin. Aspirin already thins the blood by blocking your platelets, the little cells that help form clots. Phellodendron contains berberine, and in lab and animal studies berberine seems to slow platelet clumping too. Stack those two together and, in theory, you could bleed a bit more easily, showing up as things like easy bruising, nosebleeds, bleeding gums, or stomach upset and dark stools.
I want to be honest about the strength of this: it's theoretical, based on test-tube and animal work, not solid human studies. So it's worth a conversation, not a panic. Before adding phellodendron, check with your pharmacist or doctor, especially since you're already on a daily aspirin product.
