Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist with these two together. Your TB medication, bedaquiline, gets cleared from your body by a liver enzyme called CYP3A4. Phellodendron contains berberine, which lab and early human studies suggest can partly slow that same enzyme down. So in theory, pairing them could let bedaquiline levels creep up a bit higher than expected.

Why that matters here: bedaquiline is known to affect heart rhythm (it can lengthen the QT interval), so higher levels could nudge that risk up, along with possible nausea or headache. I want to be honest though, this is a theoretical concern based on how the enzyme works, not solid proof. Since you're treating a serious infection, please don't change anything on your own. Run it by your TB doctor or pharmacist first.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
