Bisoprolol falls under ANTIHYPERTENSIVE DRUGS — the group Phellodendron interacts with.

Theoretically, phellodendron might have additive effects with antihypertensive drugs.
Phellodendron contains berberine. Animal research suggests that berberine can have hypotensive effects. Also, a clinical study suggests that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. Theoretically, phellodendron might also reduce blood pressure.

Here's the simple version: bisoprolol already lowers your blood pressure and slows your heart rate. Phellodendron contains berberine, which in animal studies and one small human trial (paired with amlodipine) seemed to nudge blood pressure down too. So stacking the two could add up, and your numbers might drop a little lower than expected.

If that happens, you may feel dizzy, lightheaded, unusually tired, or faint when you stand up. I'll be honest, this is a theoretical concern based on how these work, not strong proof, so don't panic. Still, if you take bisoprolol and want to try a phellodendron or berberine supplement, check with your pharmacist or doctor first, and keep an eye on your blood pressure at home.

Bisoprolol also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2D6.
Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Here's the gist with bisoprolol and Phellodendron. This bark contains berberine, which lab and early human studies suggest can slow down an enzyme called CYP2D6. Bisoprolol is partly cleared by that enzyme, so in theory the supplement could let a bit more drug linger in your system. Bisoprolol works as-is (it's not a prodrug that needs activating), so higher levels would mean a stronger effect.

What that might look like: more pronounced slowing of your heart rate, lower blood pressure, lightheadedness, or unusual tiredness. Keep in mind this is theoretical, not proven in real patients, so I wouldn't panic. Still, bisoprolol mostly leaves the body through other routes too. Before adding Phellodendron, please check with your pharmacist or doctor.
