Dorzolamide Hydrochloride, Timolol Maleate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2D6.
Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Cosopt PF is an eye drop that combines dorzolamide with timolol, and it's the timolol part that matters here. Timolol is a beta-blocker broken down in part by the CYP2D6 enzyme. Phellodendron contains berberine, which lab studies suggest can slow that enzyme down. In theory, that could let a bit more timolol build up in your body and cause beta-blocker type effects like a slower heart rate, lower blood pressure, tiredness, or lightheadedness.

I want to be honest that this is a theoretical concern based on how these compounds behave in the lab, not proven cases. Eye drops also deliver only small amounts into the bloodstream. Still, if you have a slow heart rate or breathing issues, it's worth a quick chat with your pharmacist or doctor before combining them.
