Phellodendron and Gabapentin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CNS DEPRESSANTS

Why this applies to Gabapentin

Gabapentin falls under CNS DEPRESSANTS — the group Phellodendron interacts with.

Theoretically, phellodendron might increase the sedative effects of CNS depressants.
Phellodendron contains berberine. Animal research suggests that berberine may have sedative effects. Theoretically, phellodendron might also have CNS depressants effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: gabapentin already tends to make people feel drowsy, foggy, or off-balance, especially when you first start it or move up a dose. Phellodendron contains berberine, and in animal studies that compound showed some calming, sedative-type activity. So the worry is simple addition. Pair the two and you could feel a bit sleepier, dizzier, or less steady on your feet than gabapentin would cause on its own.

I want to be honest about the strength of this: it's theoretical, based on how things behave in the lab and in animals, not on solid human reports. Gabapentin isn't a prodrug, so this isn't about one speeding up or blocking the other, it's just possible overlapping drowsiness. If you do take both, go easy with driving until you know how you feel, and check with your pharmacist or doctor first.

From our pharmacist’s overview on the Gabapentin page

“Gabapentin (Neurontin, Gralise) calms overactive nerves, which means it already slows your nervous system down, and that is exactly what 78 of the 106 supplements on this list also do. So the dominant risk here is sedation stacked on sedation, extra drowsiness at best and dangerously slowed breathing at worst, and with seven entries ra…”

About Phellodendron

What is Phellodendron?

Phellodendron
  • Digestive complaints and diarrhea
  • Inflammation and joint discomfort
  • Stress and weight management (in combination products)
  • Skin conditions (topical, traditional)
  • Antimicrobial support

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. Human evidence for most uses is limited, and it is often sold in combination with other herbs, making its individual effects hard to judge. Talk with your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.
