Why this applies to Pazopanib

Pazopanib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist for pazopanib (Votrient). Your body clears this cancer drug largely through a liver enzyme called CYP3A4. Phellodendron contains berberine, which lab and early human studies suggest can partly block that same enzyme. If it slows the cleanup, more pazopanib could linger in your blood, which may push up its effects and side effects.

With pazopanib that could mean more high blood pressure, diarrhea, fatigue, liver irritation, or rarely heart rhythm changes (QT prolongation). I want to be honest though: this concern is theoretical, based on how the enzyme behaves, not on real cases with these two together. Since pazopanib is a serious oncology drug, please loop in your oncologist or pharmacist before adding any phellodendron or berberine product.

Theoretically, phellodendron might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

Here's the thing to keep in mind with Votrient (pazopanib) and Phellodendron. Pazopanib itself can raise your bleeding risk, and it may also affect your platelets and blood pressure. The berberine in Phellodendron has shown, in lab and animal studies, that it can keep platelets from clumping together the way they normally do. Stack those two effects and you could, in theory, bruise or bleed a little more easily.

I want to be honest about the strength of this one. It's based on test-tube and animal research, not real-world human cases, so it's more of a caution than a proven problem. Still, because pazopanib already carries its own bleeding warnings, it's worth a quick chat with your oncologist or pharmacist before adding this herb.

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
