Phellodendron and Pentoxifylline

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Pentoxifylline

Pentoxifylline falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Phellodendron interacts with.

Theoretically, phellodendron might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with this combo: pentoxifylline already works by making your blood less thick and a bit less sticky, which helps it flow through those small vessels in your hands and feet. Phellodendron contains berberine, and in lab and animal studies berberine seems to make platelets clump together a little less too. Stack the two, and in theory you could get more of that blood-thinning effect than you bargained for.

I want to be honest, though: this is a theoretical concern based on test-tube and animal data, not solid human studies. Still, watch for things like easy bruising, nosebleeds, bleeding gums, or cuts that are slow to stop. Before adding Phellodendron, please run it by your pharmacist or doctor first.

From our pharmacist’s overview on the Pentoxifylline page

“Pentoxifylline (Trental) improves blood flow in people with poor leg circulation partly by making platelets less sticky, so treat this list as one to read carefully rather than skim. Of the 218 supplements on file, 202 can also slow clotting, which means adding them stacks bleeding risk on bleeding risk.”

About Phellodendron

What is Phellodendron?

Phellodendron
  • Digestive complaints and diarrhea
  • Inflammation and joint discomfort
  • Stress and weight management (in combination products)
  • Skin conditions (topical, traditional)
  • Antimicrobial support

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. Human evidence for most uses is limited, and it is often sold in combination with other herbs, making its individual effects hard to judge. Talk with your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.
