Deucravacitinib falls under IMMUNOSUPPRESSANTS — the group Picrorhiza interacts with.

Picrorhiza seems to have immunostimulating activity. Theoretically, picrorhiza may interfere with immunosuppressant therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the idea behind this pairing. Picrorhiza may gently rev up the immune system, while Deucravacitinib (Sotyktu) works in the opposite direction by calming an overactive immune response to clear plaque psoriasis. So in theory, this herb could push against the very thing your medication is trying to do, possibly making your skin treatment work a little less well over time.

I want to be honest about the evidence though. This concern comes only from lab and animal data, not from real people taking these two together, so it's more of a caution than a proven problem. Deucravacitinib is not a prodrug, so this isn't about how it's broken down. Still, before adding Picrorhiza, check with your pharmacist or dermatologist so your psoriasis stays well controlled.

Sotyktu (deucravacitinib) is approved to treat two conditions. First, it treats moderate-to-severe plaque psoriasis in adults who are candidates for systemic therapy or phototherapy. Second, it treats active psoriatic arthritis in adults.

Sotyktu is not recommended for use alongside other potent immunosuppressants (medicines that strongly suppress the immune system).
