Picrorhiza seems to have immunostimulating activity. Theoretically, picrorhiza may interfere with immunosuppressant therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the idea behind this pairing. Picrorhiza appears, mostly in lab and animal work, to gently rev up the immune system. Nelarabine works in a very different way (it's a chemotherapy drug that kills fast-growing cancer cells), but it also wears down your immune system as part of how it's used against leukemia and lymphoma. So in theory, an immune-stimulating herb could tug in the opposite direction of your treatment.

I want to be honest with you though: this concern is theoretical and rests on weak evidence, not human cancer studies. Still, while you're on a serious medication like nelarabine, I wouldn't add Picrorhiza without clearing it first. Please run it by your oncologist or pharmacist before using it.

What Is Nelarabine Injection Used For?

Brand name: Atriance

Nelarabine injection (brand name Arranon) is used to treat two aggressive blood cancers: T-cell acute lymphoblastic leukemia (T-ALL) and T-cell lymphoblastic lymphoma (T-LBL). It is approved for adult and pediatric patients age 1 year and older whose disease has not responded to — or has come back after — at least two prior chemotherapy regimens.

Nelarabine injection may also be given in combination with multi-agent chemotherapy as part of treatment for newly diagnosed T-ALL and T-LBL in certain settings, as studied in clinical trials.
