Ocrelizumab falls under IMMUNOSUPPRESSANTS — the group Picrorhiza interacts with.

Picrorhiza seems to have immunostimulating activity. Theoretically, picrorhiza may interfere with immunosuppressant therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the idea behind this pairing. Ocrelizumab (Ocrevus) works by quieting part of your immune system, specifically by clearing out certain B-cells, to calm the inflammation that drives MS. Picrorhiza, on the other hand, seems to stimulate the immune system in lab and animal experiments. So in theory, the herb could push in the opposite direction from your medication and blunt some of the benefit you're getting from Ocrevus.

I want to be honest that this is a theoretical concern based on test-tube and animal data, not real patient outcomes, so the actual risk is unclear. Still, since Ocrevus is a serious MS therapy, I wouldn't add Picrorhiza on your own. Please run it by your neurologist or pharmacist first before starting it.

What Is Ocrelizumab Injection Used For?

Brand name: Ocrevus

Ocrevus (ocrelizumab) is given by intravenous (IV) infusion and is approved for adults with relapsing forms of multiple sclerosis (MS) — including clinically isolated syndrome, relapsing-remitting MS, and active secondary progressive MS — as well as adults with primary progressive MS (PPMS).

Ocrevus is also approved for pediatric patients 10 years of age and older who weigh at least 25 kg (about 55 lbs) and have relapsing-remitting MS.
