Here's the gist: policosanol may gently nudge your blood sugar lower on its own, and your Metformin XR is already working to do the same thing. Stack the two and there's a chance your numbers could dip lower than expected. With metformin alone true low blood sugar is uncommon, but if policosanol adds to the effect you might notice shakiness, sweating, dizziness, or feeling unusually tired or foggy.

Keep in mind the evidence here is fairly modest, so this isn't a guaranteed problem for everyone. Still, if you decide to try policosanol, it's smart to check your sugars a bit more closely at first and have a quick chat with your pharmacist or doctor before adding it. Don't change your metformin dose on your own.

What is Policosanol?

Policosanol

Policosanol is a mix of waxy alcohols (mostly octacosanol) made from sugar cane or other plant waxes, and it is often marketed as a natural way to lower cholesterol. Early studies from Cuba showed strong cholesterol-lowering effects, but independent studies in other countries have mostly failed to confirm these benefits. Talk with your pharmacist or doctor before using it, especially if you take cholesterol medicine.
