Why this applies to Donepezil (patch)

Donepezil (patch) falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2D6.
In vitro research shows that propolis extract can inhibit CYP2D6. However, animal research shows that propolis extract does not significantly affect CYP2D6 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2D6 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the short version if you're using the donepezil (Adlarity) patch for Alzheimer's along with propolis: your body partly clears donepezil through a liver enzyme called CYP2D6. In test-tube studies, propolis can slow that enzyme down. In theory, that could let donepezil build up a bit and lead to more of its usual side effects, like nausea, diarrhea, a slower heartbeat, dizziness, trouble sleeping, or muscle cramps.

The reassuring part is that this is only theoretical. Animal studies didn't show a real effect, and it hasn't been reported in people. Everyday amounts of propolis are unlikely to cause trouble, though very concentrated supplement doses are less predictable. If you'd like, run it by your pharmacist or doctor so they can keep an eye out.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. Here's the situation with your donepezil patch (Adlarity) and propolis. Donepezil is cleared partly by a liver enzyme called CYP3A4. In a test tube, some propolis extracts seem to slow that enzyme down, which in theory could let donepezil build up a bit more than expected. If that happened, you might notice more of the typical donepezil side effects, like nausea, diarrhea, poor appetite, dizziness, or a slower heartbeat.

The good news: this is very shaky evidence. It comes from lab and animal studies, not people, and other lab tests showed no effect at all. Everyday amounts of propolis are unlikely to cause trouble. Still, if you're taking concentrated propolis supplements, it's worth a quick chat with your pharmacist or doctor rather than changing anything yourself.
