Why this applies to Selpercatinib

Selpercatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the basic idea with Retevmo (selpercatinib): your liver uses an enzyme called CYP3A4 to clear it from your body. Some lab studies hint that propolis might slow that enzyme down. If that happened, more selpercatinib could linger in your blood, which in theory could mean stronger side effects like high blood pressure, liver enzyme changes, diarrhea, swelling, or heart rhythm changes (a longer QT interval).

That said, I want to keep this in perspective. This concern comes only from test-tube and animal research, and even those results disagree with each other. It has never actually been reported in people, and propolis is poorly absorbed, so any real-world effect is likely small. Still, because this is a serious cancer medicine, please check with your oncologist or pharmacist before adding propolis.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
