There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.

Here's the simple version: this isn't about gotu kola changing how your body handles selpercatinib. It's about both being potentially hard on the liver. Selpercatinib (Retevmo) can raise liver enzymes and, in some people, cause more serious liver injury, which is why your care team checks bloodwork regularly while you're on it. Gotu kola has been tied to a handful of liver injury reports too, though the evidence is thin and it's not proven the herb was the real cause.

The concern is simply adding one liver stressor on top of another. Watch for things like yellowing of the eyes or skin, dark urine, belly pain, or unusual tiredness. Please don't start or stop anything on your own. Loop in your oncology team or pharmacist before adding gotu kola.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
