Here's the concern with pairing ashwagandha and selpercatinib (Retevmo). This isn't about one speeding up or slowing down the other. Instead, both can be hard on your liver, so taking them together may add up and raise your risk of liver trouble. Selpercatinib is already known to bump up liver enzymes in some people, and ashwagandha has been tied to rare but real cases of liver injury.

I'll be honest, this is a theoretical caution based on how each affects the liver, not on studies of the two combined. Still, since you're already on a serious cancer medicine that needs liver monitoring, I'd hold off on ashwagandha and check with your oncologist or pharmacist first. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness.

Here's the gist for selpercatinib (Retevmo): your body relies on a liver enzyme called CYP3A4 to handle this medicine. Lab studies hint that ashwagandha may speed up that enzyme, which in theory could clear selpercatinib a little faster and leave slightly less in your bloodstream. For a cancer drug, that could mean it isn't working as fully as it should, which is something you really don't want to risk.

I want to be honest about the strength of this, though. This is based only on test-tube research, not on people, so it's a theoretical concern rated as minor. Still, with a serious cancer medicine I'd play it safe. Please loop in your oncologist or pharmacist before adding ashwagandha, and don't stop or change anything on your own.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
