Here's the concern in simple terms: selpercatinib (Retevmo) can be hard on your liver, and it's monitored with regular blood tests during treatment. Turmeric, especially concentrated supplements made to absorb better, has been linked in some reports to liver irritation too. Stacking two things that stress the same organ may, in theory, raise the chance of liver problems like fatigue, nausea, belly pain, dark urine, or yellowing of the skin or eyes.

I want to be honest that this is based on anecdotal reports, not strong studies, and the turmeric you sprinkle on food is very unlikely to be an issue. The caution is really about high-dose pills. Since selpercatinib is a serious cancer medicine, please check with your oncologist or pharmacist before adding any turmeric supplement.

Selpercatinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Turmeric might increase or decrease levels of drugs metabolized by CYP3A4.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

Here's the gist: your cancer medicine, selpercatinib (Retevmo), is cleared from your body mainly by a liver enzyme called CYP3A4. Turmeric, especially in concentrated supplement form, may interfere with that enzyme. The lab and animal data point in two directions. Most often turmeric appears to slow CYP3A4, which could let selpercatinib build up and bring on more side effects like nausea, diarrhea, swelling, high blood pressure, or liver irritation. Less commonly it may speed the enzyme up, which could weaken the drug.

This is based on weak evidence (mostly test-tube and animal work), so don't panic. The amount you sprinkle on food is unlikely to matter, but a daily curcumin pill might. Please check with your oncologist or pharmacist before adding turmeric supplements.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
