Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of cannabis.

Here the spotlight is actually on the cannabis side of things. Your selpercatinib (Retevmo) can block an enzyme called CYP3A4, and that same enzyme helps your body break down THC, the part of cannabis that gets you high. So in theory, taking them together could let THC build up a bit, making cannabis hit harder or last longer. That could mean more drowsiness, dizziness, a faster heartbeat, or feeling more impaired than you expected.

Keep in mind this is a theoretical concern based on how these chemicals are processed, not something proven in studies. Still, since cancer treatment is serious business, I'd want you to mention any cannabis use to your oncologist or pharmacist so they can keep an eye on side effects. Please don't change your selpercatinib on your own.

Selpercatinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist for Retevmo (selpercatinib): your body relies on a liver enzyme called CYP3A4 to clear it out. Lab studies suggest cannabis (especially CBD-heavy products) can slow that enzyme down a bit. If clearance slows, selpercatinib levels could creep up, which may mean more of its known side effects, things like high blood pressure, liver enzyme changes, swelling, diarrhea, dry mouth, or fatigue.

The honest part: this is theoretical, based only on test-tube research, not real patient cases, so the effect may be small or not show up at all. Still, since selpercatinib is a serious cancer medicine where dosing really matters, please loop in your oncologist or pharmacist before adding any cannabis product so they can keep an eye on you.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
