Why this applies to Amiodarone

Amiodarone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the situation with these two. Your body uses an enzyme called CYP3A4 to help break down amiodarone, and lab studies suggest quercetin can interfere with that enzyme. In most of those studies quercetin slowed the enzyme down, which in theory could let amiodarone build up a bit higher than expected. Amiodarone is an active drug (not a prodrug), so more of it usually means stronger effects and a greater chance of side effects like a very slow heartbeat, dizziness, fatigue, or low blood pressure.

I'll be honest though, this is mostly test-tube and animal evidence, and one human study actually showed the opposite. Amiodarone is also a tricky, narrow-margin drug. Small everyday food amounts of quercetin are unlikely to matter, but please check with your pharmacist or doctor before using a high-dose supplement.

Amiodarone also falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C8 substrates.

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here's the gist for amiodarone and quercetin. Amiodarone is partly cleared by a liver enzyme called CYP2C8, and in test-tube studies quercetin can slow that enzyme down. In theory, blocking that pathway could let amiodarone build up a bit more in your system, which might nudge up side effects like a very slow heartbeat, dizziness, tiredness, or thyroid and liver changes over time.

I want to be honest about the evidence, though: this is based on lab work, not real patients, and a human study with a similar drug showed no meaningful effect. So the actual risk is likely small. Still, amiodarone is a powerful heart-rhythm drug with a narrow safety window, so before adding a high-dose quercetin supplement, please check with your pharmacist or cardiologist first. Food amounts are not a concern.

What Is Amiodarone Used For?

Amiodarone is prescribed for adults with serious, life-threatening heart-rhythm problems that haven't responded to other medications. Specifically, it's used for two dangerous arrhythmias (abnormal heartbeats): recurrent ventricular fibrillation (a chaotic, potentially fatal heart rhythm) and recurrent ventricular tachycardia (a dangerously fast heart rhythm that causes unstable blood pressure).

Because amiodarone comes with significant risks, doctors reserve it for situations where other options haven't worked or can't be tolerated. It is not a first-choice medication — it's a last-resort tool for serious, life-threatening conditions.
