In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.

Here's the short version on combining quercetin with Fiorinal with Codeine. Quercetin can block a kidney transporter called OAT3, which helps your body flush out certain drugs. The aspirin part of this combo product is a known OAT3 substrate, so in theory, quercetin could slow how quickly aspirin leaves your system, letting a bit more build up. That might mean a slightly higher chance of aspirin's usual side effects, like stomach upset, heartburn, or, at higher levels, ringing in the ears.
Now the reassuring part: this is based only on lab studies, and it has not been seen in real people. Normal food amounts of quercetin are very unlikely to matter. If you're taking high-dose supplements, just check with your pharmacist or doctor first.

Here's the gist with Fiorinal with Codeine, which is a combo of aspirin, butalbital, caffeine, and codeine. Your kidneys use little protein doorways called OAT1 transporters to help clear certain drugs, including the aspirin part of this medicine. In test-tube studies, quercetin slowed those doorways down. In theory, that could let a bit more of the drug linger in your body, nudging up effects and side effects like stomach upset, ringing in the ears, or extra drowsiness.
I want to be straight with you though: this is only lab-dish data, and it has never actually been seen in real people. Everyday food amounts of quercetin are very unlikely to matter. If you take high-dose supplements, just run it by your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.

Here's the key thing about Fiorinal with Codeine: the codeine part is a prodrug. Your body needs an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves pain. Quercetin appears to block that enzyme, at least in lab studies. So in theory, less codeine would get turned into morphine, which could mean weaker pain relief rather than more side effects.
That said, this comes only from test-tube research and hasn't been shown in real people, so I wouldn't lose sleep over it. Food amounts of quercetin (apples, onions, berries) are very unlikely to matter. If you take high-dose supplements and notice your pain medicine isn't working as well, just check in with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

This is a combination pill, and the part that matters most here is the butalbital, a barbiturate that your liver clears partly through an enzyme called CYP3A4. Quercetin can nudge that enzyme in lab studies, but the human data are mixed. In some people it seems to block the enzyme (which could let butalbital linger and add to drowsiness, dizziness, or grogginess), while in others it actually sped up clearance. So we honestly can't predict the direction with confidence.
The good news is the evidence is weak, mostly test-tube and animal work, and quercetin from foods is very unlikely to matter. With concentrated high-dose supplements, I'd just check with your pharmacist or doctor first, since this product already carries sedation and dependence concerns on its own.
