Aspirin, Codeine Phosphate falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.
In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version for your aspirin with codeine: quercetin can block a kidney transporter called OAT3, which helps clear certain drugs (including the aspirin part) out through your urine. If that exit door is slowed, a bit more of the drug could linger in your body. In theory, that might mean slightly stronger effects or more side effects, like stomach upset, ringing in the ears, or easier bruising from the aspirin side.
But here's the important part: this is based only on lab (test tube) research, and it has never actually been reported in real people. Food amounts of quercetin almost certainly won't matter. If you're considering high-dose quercetin supplements, just run it by your pharmacist or doctor first.

Here's the gist for your aspirin with codeine combo. Your kidneys use a transporter called OAT1 to help clear out salicylate, the active piece of aspirin. In lab dish studies (not in real people yet), quercetin blocks that transporter. The idea is that if aspirin gets cleared a little slower, more could linger in your system, which could nudge up aspirin-type side effects like stomach upset, heartburn, ringing in the ears, or easier bruising.
I want to be honest though: this is theory based only on test-tube work, and it has never been reported in actual patients. Food amounts of quercetin are very unlikely to matter. If you're considering high-dose supplements, just run it by your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the twist with this combo: the codeine part of your pain pill is actually a prodrug. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves pain. Lab studies suggest quercetin can block that same enzyme. In theory, less conversion means less pain relief, not more. The aspirin part isn't affected by this.
The good news? This has only been seen in test-tube research, never confirmed in real people, so I wouldn't lose sleep over it. Food amounts of quercetin (apples, onions, tea) are very unlikely to matter. If you take high-dose supplements and notice your pain isn't well controlled, just mention it to your pharmacist or doctor.
