Here's the gist with this combo: one piece of your medicine, the aspirin part, gets cleared by the kidneys with help from a transporter called OAT3. In a lab dish, quercetin blocks that transporter pretty strongly. The thought is that if aspirin leaves your body a little slower, more of it could hang around, which might mean more aspirin-type effects like stomach upset, heartburn, or a higher chance of bleeding or bruising.

I want to be straight with you, though: this is purely theoretical. It has only been seen in test tubes, never reported in actual people, and the hydrocodone side isn't really the concern here. Everyday food amounts of quercetin are unlikely to matter. Before starting a high-dose supplement, just run it by your pharmacist or doctor.

This combo product pairs aspirin with hydrocodone, an opioid pain reliever. The piece that touches this interaction is the aspirin side. Aspirin (as salicylate) is partly cleared by your kidneys using a transporter called OAT1. In lab dish studies, quercetin can block OAT1. If that blocking happened in real life, your body might clear aspirin a little slower, letting it build up. That could mean more aspirin-type effects like stomach upset, heartburn, ringing in the ears, or easier bruising and bleeding.

This combo pill pairs aspirin with hydrocodone, and it's the hydrocodone part that matters here. Hydrocodone is partly turned into a stronger active painkiller (hydromorphone) by a liver enzyme called CYP2D6. In lab dish studies, quercetin can slow that enzyme down. If that happened in real life, it could shift how much active drug you make, so the practical effect on pain relief and side effects is hard to predict.

Here's the honest part: this is only test-tube data and has not been seen in actual people. Food amounts of quercetin are very unlikely to matter. Still, with an opioid, watch for things like extra drowsiness, dizziness, or slowed breathing. Before starting high-dose quercetin supplements, check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist for this combo pill. The hydrocodone part is actually a prodrug: your liver enzymes (including CYP3A4 and CYP2D6) convert part of it into stronger, more active forms. Quercetin can nudge CYP3A4 in either direction, so the lab data is genuinely mixed. If it slows that enzyme, hydrocodone could linger a bit longer, which in theory means more drowsiness, dizziness, constipation, or slowed breathing. If it speeds things up, you might just feel a little less pain relief.

The honest part: this is based on lab and animal studies plus a few small human trials, not solid evidence with this exact medicine. Everyday food amounts of quercetin almost certainly won't matter. Before starting a high-dose supplement, run it by your pharmacist or doctor.
