Why this applies to Atazanavir, Cobicistat

Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist with Evotaz (atazanavir plus cobicistat). Both pieces of this combo lean on the liver enzyme CYP3A4. Cobicistat is actually a deliberate blocker of that enzyme, and it's added on purpose to keep atazanavir levels high enough to fight HIV. Quercetin tinkers with the same enzyme, but the lab data point in two different directions: some studies suggest it slows CYP3A4 (which could nudge drug levels up, meaning more chance of side effects like nausea, headache, or yellowing of the eyes from raised bilirubin), while one human study hinted it might speed the enzyme up (which could lower atazanavir levels and let the virus gain ground).

The honest takeaway is that this is mostly theoretical and based on lab and animal work, not solid human studies with this exact drug. Food amounts of quercetin almost certainly don't matter. Still, because keeping HIV controlled is so important, please check with your pharmacist or doctor before adding a concentrated quercetin supplement.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
