CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Chlorpheniramine
Chlorpheniramine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses a liver enzyme called CYP3A4 to help clear chlorpheniramine. Quercetin, in supplement form, may slow that enzyme down a bit. If it does, a little more chlorpheniramine could hang around in your system, which might nudge up the usual side effects you'd expect from an antihistamine like this one, things like drowsiness, dry mouth, blurry vision, or feeling foggy.
I want to be honest about how shaky this evidence is. It's based mostly on lab and animal studies, and quercetin's effect on this enzyme has actually gone both directions in research. Food amounts (apples, onions, berries) are very unlikely to matter. If you're taking concentrated quercetin capsules, just mention it to your pharmacist or doctor before combining them.

Chlorpheniramine is used to temporarily relieve common allergy symptoms caused by hay fever (allergic rhinitis) or other upper respiratory allergies. These include runny nose, sneezing, itchy or watery eyes, and itching of the nose or throat.
These uses are supported across multiple products — including tablets (such as Aller-chlor, Allergy, Allergy Relief, Allergy Time, Anti Allergy, and Chlorpheniramine Maleate), extended-release tablets (Allergy Relief and Chlorphen-12), the oral solution Ed Chlorped Jr, and the inhalers ClorRelief and ClorRelief KIDs.
