Arthrotec is a combo of diclofenac (an anti-inflammatory pain reliever) and misoprostol (added to protect your stomach). Your kidneys help clear diclofenac partly through a transporter called OAT3. In test-tube studies, quercetin blocks that transporter pretty strongly. The idea is that if OAT3 is slowed down, a bit more diclofenac could hang around in your body, which could nudge up its effects and side effects like stomach upset, fluid retention, higher blood pressure, or kidney strain.

Here's the honest part: this is only based on lab work, and it hasn't actually been seen in real people. Food amounts of quercetin are very unlikely to matter. If you're thinking about high-dose quercetin supplements alongside Arthrotec, just run it by your pharmacist or doctor first.

Your kidneys use a transporter called OAT1 to help clear diclofenac from your body. In lab dish studies, quercetin blocks that transporter. In theory, if diclofenac is cleared a little slower, more could linger in your system, which could nudge up its usual side effects like stomach upset, swelling, raised blood pressure, or stress on the kidneys.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

The piece that matters here is the diclofenac, since your liver clears it using an enzyme called CYP2C9. Quercetin appears to slow that enzyme down, so the diclofenac can clear out more slowly and build up. In one small study, high-dose quercetin (500 mg twice daily) raised diclofenac blood levels by about 75 percent.

In plain terms, more diclofenac in your system could mean more of its usual side effects, like stomach upset, heartburn, fluid retention, higher blood pressure, or kidney strain. This is a moderate, theoretical concern from limited research, and food amounts of quercetin are unlikely to matter. Quercetin in concentrated supplement doses appears to slow that enzyme down, so the diclofenac doesn't get broken down as fast and can build up. In one small study, quercetin pushed diclofenac blood levels up by about 75 percent.

In real terms, more diclofenac in your system can mean more of its usual downsides: stomach upset or irritation, a higher risk of ulcers or bleeding, fluid retention, and added strain on the kidneys. The amounts in everyday foods aren't likely to matter much, but high-dose supplements could. Please check with your pharmacist or doctor before pairing the two.
