P-GLYCOPROTEIN SUBSTRATES

drospirenone, ethinyl estradiol, levomefolate falls under P-GLYCOPROTEIN SUBSTRATES — the group Quercetin interacts with.
There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist for your Beyaz birth control pill. Quercetin, especially in concentrated supplement doses, may block a transporter in your gut called P-glycoprotein. Since the hormones in Beyaz are carried by this same pump, blocking it could let a bit more of the medication into your bloodstream. In plain terms, your hormone levels might run slightly higher than usual.
What might that feel like? Possibly more of the common side effects, like nausea, breast tenderness, bloating, or mood changes. The good news is this is mostly theoretical and based on small, mixed studies, so it may not amount to much. Eating quercetin-rich foods like apples and onions is very unlikely to matter. Before starting high-dose supplements, just check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2drospirenone, ethinyl estradiol, levomefolate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Beyaz is a birth control pill that combines drospirenone, ethinyl estradiol, and a form of folate. The estrogen part is partly processed by a liver enzyme called CYP3A4, and quercetin can nudge that enzyme in both directions depending on the study, so this is mostly a theoretical concern based on lab and animal work, not solid human pill data.
If quercetin slowed that enzyme, hormone levels could run a bit higher, which might mean more nausea, breast tenderness, or spotting. If it sped the enzyme up instead, hormone levels could dip, and in theory that could weaken pregnancy protection. The everyday amounts in foods are very unlikely to matter. Before starting a high-dose quercetin supplement, please check with your pharmacist or doctor.
