P-GLYCOPROTEIN SUBSTRATES
Why this applies to Estradiol, Norgestimate
Estradiol, Norgestimate falls under P-GLYCOPROTEIN SUBSTRATES — the group Quercetin interacts with.
There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist with Ortho-Prefest (estradiol plus norgestimate) and quercetin. These hormones are partly handled by a transporter in your gut called P-glycoprotein, which acts like a bouncer that pushes some drug back out before it gets absorbed. Quercetin may slow that bouncer down, so in theory a little more hormone could slip into your bloodstream and raise your levels.
If that happened, you might notice more of the usual estrogen-type effects, like breast tenderness, nausea, bloating, headaches, or spotting. I want to be honest though: this is mostly theoretical and based on small, mixed studies, not strong proof. Everyday food amounts of quercetin are very unlikely to matter. If you're considering high-dose supplements, just run it by your pharmacist or doctor first.
