Why this applies to Gatifloxacin

Gatifloxacin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Quercetin interacts with.

Theoretically, concomitant use might increase the effects and adverse effects of OATP substrates.

In vitro evidence shows that quercetin can inhibit organic anion-transporting peptide (OATP) 1B1-mediated uptake of estrone-3-sulfate and pravastatin. Furthermore, clinical research in healthy males shows that intake of quercetin along with pravastatin increases the AUC of pravastatin by 24%, prolongs its half-life by 14%, and decreases its apparent clearance by 18%, suggesting that quercetin modestly inhibits the uptake of pravastatin in hepatic cells.

Here's the gist: quercetin can gently block a transporter called OATP that helps move certain medicines into your cells. Gatifloxacin (Tequin) is an antibiotic that's at least partly handled by these transporters, so in theory taking concentrated quercetin supplements alongside it could let a bit more antibiotic build up in your body. That could mean a slightly stronger effect and a higher chance of side effects like nausea, headache, dizziness, or, more notably with this drug class, changes in blood sugar.

I want to be honest, though: this is mostly theoretical. The real human data we have is for pravastatin, not gatifloxacin, and the change there was modest. Normal food amounts of quercetin almost certainly won't matter. If you're considering a high-dose supplement, just run it by your pharmacist or doctor first.

Here's the gist: Gatifloxacin is a fluoroquinolone antibiotic that kills bacteria by jamming up an enzyme the germs need called DNA gyrase. The concern with quercetin is that, in test tube studies, it likes to park itself on that very same spot on the bacteria. If quercetin gets there first, the worry is that it could compete with the antibiotic and make it work a little less well, which in theory means a weaker punch against your infection.

I want to be honest, though, this is mostly lab-dish science and not strongly proven in people. The amount of quercetin you'd get from foods like onions or apples is very unlikely to matter. If you're taking concentrated quercetin pills while on Gatifloxacin, just give your pharmacist or doctor a quick heads-up so they can confirm you're getting the full benefit of your antibiotic.
