Quercetin interacts with whole categories of medication. Glecaprevir, Pibrentasvir falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Glecaprevir, Pibrentasvir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Mavyret (glecaprevir and pibrentasvir) is partly handled by a liver enzyme called CYP3A4, and quercetin can nudge that enzyme. The tricky part is that the research points in both directions. In some studies quercetin slows CYP3A4, which could let more of the drug build up. In another study it actually sped the enzyme up. So we can't say with confidence which way it would push your Mavyret levels.
The good news is that this concern comes from lab and animal work plus a few small human studies, not from people taking these two together, so it's fairly theoretical. Food amounts of quercetin (in apples, onions, tea, and the like) are very unlikely to matter. Before adding a high-dose quercetin supplement during your hepatitis C treatment, please run it by your pharmacist or doctor first.

P-GLYCOPROTEIN SUBSTRATES
2 of 2Glecaprevir, Pibrentasvir also falls under P-GLYCOPROTEIN SUBSTRATES.
There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist for Mavyret (glecaprevir and pibrentasvir): both of these antiviral ingredients are carried by a transporter in your gut called P-glycoprotein. Quercetin, in concentrated supplement doses, may block that pump a bit. In theory, that could let more of the drug get absorbed, nudging your blood levels up and possibly raising the chance of side effects like headache, tiredness, nausea, or itching.
I want to be honest though, this is mostly lab and small-study evidence, and the results have been inconsistent, so it's far from certain it would matter in real life. The quercetin you get from foods like apples, onions, and berries is very unlikely to cause a problem. If you're thinking about a high-dose supplement while finishing your hepatitis C course, run it by your pharmacist or doctor first so your treatment stays on track.

What Is Glecaprevir and Pibrentasvir Used For?
Brand name: Mavyret
Mavyret treats acute or chronic hepatitis C virus (HCV) infection caused by genotypes 1, 2, 3, 4, 5, or 6 in adults and children 3 years and older who have no cirrhosis or have compensated cirrhosis (mild liver scarring that is still functioning).
Mavyret is also approved for adults and children 3 years and older with genotype 1 HCV who were previously treated with either an NS5A inhibitor or an NS3/4A protease inhibitor (two earlier classes of hepatitis C drugs) — but not both at the same time.
