Hydrocodone, Ibuprofen falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.
In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Vicoprofen, which combines hydrocodone (an opioid pain reliever) and ibuprofen (an anti-inflammatory). Your kidneys use a transporter called OAT3 to help clear certain drugs out of your body. In lab dish studies, quercetin blocks that transporter pretty strongly. The thought is that if clearance slows down, a bit more of the drug could hang around, which in theory could nudge up effects and side effects like drowsiness, constipation, stomach upset, or fluid retention.
The important part: this has only been seen in test tube research, never in real people. Food amounts of quercetin are very unlikely to matter. If you're considering high-dose supplements, just run it by your pharmacist or doctor first.

Here's something worth knowing about Vicoprofen, which combines hydrocodone with ibuprofen. The hydrocodone part actually relies on an enzyme called CYP2D6 to turn into its stronger painkilling form, hydromorphone. In a test-tube setting, quercetin appears to slow that enzyme down. If that happened in the body, it could mean hydrocodone is converted less, so you might get a bit less pain relief than expected.
I want to be honest, though: this is only seen in lab studies and has not been shown in real people, so it's largely theoretical. Food amounts of quercetin are very unlikely to matter. If you take high-dose supplements and notice your pain isn't well controlled, just check in with your pharmacist or doctor before changing anything.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Vicoprofen is a combo product, and the part that matters here is the ibuprofen. Your liver uses an enzyme called CYP2C9 to clear ibuprofen out of your body. High-dose quercetin can slow that enzyme down, so the ibuprofen may hang around a bit longer and build up slightly. That could nudge up the usual ibuprofen side effects, things like stomach upset, heartburn, an irritated or bleeding stomach lining, and over time more strain on the kidneys.
Keep in mind this is mostly theoretical, drawn from a small study on a similar drug and lab work, not direct testing with ibuprofen. Quercetin from foods is very unlikely to matter, but concentrated supplements could. If you take both regularly, just check in with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the deal with Vicoprofen, which combines hydrocodone (an opioid pain reliever) with ibuprofen. The hydrocodone part is partly handled by an enzyme called CYP3A4, and quercetin can nudge that enzyme. The tricky part is that quercetin seems to pull in both directions depending on the study, sometimes blocking the enzyme and sometimes speeding it up. So we honestly can't predict exactly which way it would go for you.
If quercetin slowed that enzyme, more hydrocodone could hang around, possibly meaning extra drowsiness, dizziness, or constipation. The good news is this is based on lab and theoretical work, not solid human studies with this drug, and food amounts of quercetin are unlikely to matter. If you're taking high-dose supplements, just run it by your pharmacist or doctor.
