Why Are There 2 Interactions?
Quercetin interacts with whole categories of medication. Metformin, Linagliptin falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
P-GLYCOPROTEIN SUBSTRATES

Metformin, Linagliptin falls under P-GLYCOPROTEIN SUBSTRATES — the group Quercetin interacts with.
There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the deal with Jentadueto (which combines metformin and linagliptin) and quercetin. The linagliptin part is moved around the body by a transporter called P-glycoprotein, a kind of cellular pump. There's early, fairly weak evidence that quercetin can block this pump, which in theory might let a little more linagliptin get absorbed and raise its levels.
If that happens, you could be a touch more likely to feel side effects, though linagliptin is usually gentle. By itself it rarely causes low blood sugar, but combined with metformin you might notice mild dizziness, nausea, or stomach upset. Honestly, this is mostly theoretical and based on small studies. Food amounts of quercetin almost certainly don't matter. Before starting a high-dose supplement, just run it by your pharmacist or doctor.

ANTIDIABETES DRUGS

Clinical research suggests that a combination of quercetin, myricetin, and chlorogenic acid reduce levels of fasting glucose in patients with type 2 diabetes, including those already taking antidiabetes agents. The effect of quercetin alone is unknown.

Jentadueto combines two diabetes medicines, metformin and linagliptin, both of which work to lower your blood sugar. The concern here is fairly simple: quercetin may have a mild blood-sugar-lowering effect of its own. So if you stack a high-dose quercetin supplement on top of your Jentadueto, your levels could theoretically dip lower than expected.
Honestly, the evidence is thin. The one study that showed an effect mixed quercetin with other compounds, so we don't really know what quercetin alone does. Quercetin from food is very unlikely to matter. If you take a concentrated supplement, just keep an eye out for shakiness, sweating, or lightheadedness, and check in with your pharmacist before starting it.
