Mometasone, formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.
In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest version: this concern comes from lab dish studies showing quercetin can block a liver enzyme called CYP2D6. That hasn't actually been seen happen in people, so think of it as a theoretical flag, not a proven problem.
With your Zenhale inhaler, the formoterol part is the piece that touches CYP2D6 a bit. In theory, if quercetin slowed that breakdown, slightly more formoterol could linger and you might notice things like a faster or pounding heartbeat, jitteriness, shaky hands, or a headache. The mometasone steroid side isn't really affected this way. Food amounts of quercetin almost certainly don't matter. If you're using high-dose supplements, just run it by your pharmacist or doctor first.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the honest take on combining quercetin with your Zenhale inhaler. The interaction on file is about quercetin slowing down a liver enzyme called CYP2C9, which could let certain drugs build up. The catch is that neither mometasone nor formoterol, the two medicines in Zenhale, really relies on CYP2C9. They're processed through other pathways, so this particular concern doesn't line up neatly with your inhaler.
On top of that, the evidence here is mostly theoretical and based on other drugs like diclofenac, not inhaled steroids or beta-agonists. So I wouldn't lose sleep over it. Still, if you're using high-dose quercetin supplements (not just food amounts), it's worth a quick word with your pharmacist to be safe.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Zenhale combines two active drugs you breathe in: mometasone (an inhaled steroid) and formoterol (a long-acting bronchodilator that opens your airways). Both are partly broken down by a liver enzyme called CYP3A4. Quercetin may slow that enzyme down, which could let a little more of these drugs hang around in your body. In theory, that might nudge up steroid-related effects or formoterol side effects like a faster heartbeat, jitteriness, shakiness, or a headache.
Here's the honest part: this is based mostly on lab and animal research, and quercetin's effect on this enzyme has been inconsistent in people. Inhaled doses also reach the bloodstream in fairly small amounts. Food amounts of quercetin are unlikely to matter, but before starting high-dose supplements, check with your pharmacist or doctor.
