CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Nintedanib Esylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist for Ofev (nintedanib): your liver and gut use an enzyme called CYP3A4 to help clear this drug. Quercetin, in concentrated supplement doses, may slow that enzyme down. If that happens, more nintedanib could hang around in your body, which may bump up its usual side effects like diarrhea, nausea, vomiting, belly pain, and changes in your liver tests.
I want to be honest about the evidence here. This is mostly lab and animal data, and the results are actually mixed (some studies even hint quercetin could speed CYP3A4 up). Nintedanib is not a prodrug, so it doesn't get flipped. Quercetin in everyday food amounts is unlikely to matter. Before adding a high-dose supplement, please check with your pharmacist or doctor.

P-GLYCOPROTEIN SUBSTRATES

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist: your lung medication Ofev (nintedanib) relies on a cellular pump called P-glycoprotein to control how much actually gets absorbed and stays in your body. Quercetin, in concentrated supplement doses, may block that pump. If it does, more nintedanib could end up in your bloodstream than expected, which can mean stronger side effects like nausea, stomach upset, diarrhea, vomiting, or changes in your liver enzymes.
I want to be honest that this is mostly theoretical for nintedanib specifically, based on lab work and a few small human studies of other drugs, not on nintedanib itself. Everyday food amounts of quercetin are unlikely to be a problem. Before starting a high-dose quercetin supplement, please run it by your pharmacist or doctor first.

What Is Nintedanib Used For?
Brand name: Ofev
Nintedanib treats certain scarring lung diseases in adults. All the supported products are indicated for idiopathic pulmonary fibrosis (IPF) and for chronic fibrosing interstitial lung diseases (ILDs) with a progressive phenotype (scarring lung disease that keeps getting worse).
Ofev, nintedanib capsules (100 mg and 150 mg) and one other nintedanib capsule product are also indicated to slow the decline in lung function in systemic sclerosis-associated interstitial lung disease (SSc-ILD). The nintedanib esylate capsule label lists only IPF and progressive chronic fibrosing ILDs.
