Why this applies to Panobinostat

Panobinostat falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the effects and adverse effects of OAT3 substrates.

In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.

Here's the idea behind this one. Your kidneys use a transporter called OAT3 to help move certain drugs out of your body. In test-tube studies, quercetin appears to block OAT3 fairly strongly. Since panobinostat (Farydak) is handled in part by that transporter, the theory is that high-dose quercetin could slow its clearance and let a bit more of the drug linger in your system.

If that happened, you might see more of panobinostat's usual side effects, like diarrhea, nausea, fatigue, low blood counts, or heart rhythm changes. I want to be honest though: this is only laboratory data and has never been confirmed in people. Food amounts of quercetin are very unlikely to matter. Because panobinostat is serious cancer therapy, please run any quercetin supplement past your oncology team first.

Panobinostat also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the situation with quercetin and your Panobinostat (Farydak). Your liver uses an enzyme called CYP3A4 to help break down and clear panobinostat from your body. Quercetin can tug on that same enzyme. In lab and a few small human studies it mostly seems to slow CYP3A4, which could let more panobinostat build up. With this drug, higher levels can mean worse diarrhea, nausea, fatigue, low blood counts, and heart rhythm changes (a longer QT interval).

Honestly, the evidence here is thin and mixed, mostly lab and animal work, and one study even hinted quercetin might speed CYP3A4 up. Food amounts are unlikely to matter, but high-dose supplements during cancer treatment deserve a quick check. Please run any quercetin supplement past your oncology team or pharmacist first.

  • Panobinostat is used in combination with bortezomib (Velcade) and dexamethasone to treat people with multiple myeloma (a type of cancer of the bone marrow) who have already been treated two other medications, including bortezomib (Velcade). Panobinostat is in a class of medications called histone deacetylase (HDAC) inhibitors. It works by killing cancer cells.
