CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Perampanel falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Fycompa (perampanel). Your liver helps clear this seizure medicine, and quercetin in higher supplement doses may slow down one of the enzymes (CYP2C9) involved in that process. If perampanel gets cleared more slowly, it can build up in your body. Since this drug is active as-is (not a prodrug), that could mean stronger effects and more side effects, like dizziness, drowsiness, trouble walking, or mood and behavior changes.
To be honest, this is mostly theoretical for perampanel. The clearest evidence comes from a different drug, and high-dose quercetin matters far more than the small amounts in foods like apples and onions. Still, with seizure meds it's worth a quick chat with your pharmacist or doctor before adding a concentrated supplement.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your body clears Perampanel mainly through a liver enzyme called CYP3A4, and quercetin can tug on that same enzyme. The tricky part is the research pulls in two directions. In some lab and human studies quercetin slows CYP3A4, which could let perampanel build up and bring on more drowsiness, dizziness, trouble with balance, or irritability. In another study it actually sped the enzyme up, which could weaken perampanel and make seizures harder to control.
Honestly, this is mostly theoretical and based on weak, mixed evidence, and food amounts of quercetin are unlikely to matter. But because seizure control is too important to gamble with, please run any high-dose quercetin supplement past your pharmacist or doctor first.

What Is Perampanel Used For?
Brand name: Fycompa
Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.
Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
