Why this applies to Sofosbuvir, Velpatasvir

Sofosbuvir, Velpatasvir falls under P-GLYCOPROTEIN SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist for your Epclusa (sofosbuvir and velpatasvir). Both of these antivirals are carried in and out of your gut cells by a little pump called P-glycoprotein. Quercetin, in concentrated supplement doses, may slow that pump down a bit. In theory, that could let a little more of the drug get absorbed, which might raise your blood levels and the chance of mild side effects like headache, tiredness, or nausea.

I want to be honest though: this is based on early, mixed lab and small human studies, not solid trials with Epclusa itself, so it's more of a caution than a real red flag. The quercetin you get from foods is very unlikely to matter. Since finishing your full hepatitis C course is what cures you, just check with your pharmacist or doctor before adding a high-dose quercetin supplement.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Sofosbuvir, Velpatasvir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist of it for your Epclusa (sofosbuvir/velpatasvir). One of its ingredients, velpatasvir, is partly handled by a liver enzyme called CYP3A4, and quercetin can nudge that enzyme in either direction depending on the study. So in theory, a high-dose quercetin supplement could change how much velpatasvir is in your blood, either raising it (more chance of headache, tiredness, or nausea) or lowering it slightly.

That said, this concern comes from lab and animal work plus a few small studies, not from people taking Epclusa, so it's quite uncertain. Food amounts of quercetin (apples, onions, berries) are very unlikely to matter. Since finishing your full hepatitis C course exactly as prescribed is what cures you, please check with your pharmacist or doctor before adding a quercetin supplement.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C8 substrates.

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here's the gist if you're taking Epclusa (sofosbuvir/velpatasvir) for hepatitis C and thinking about a quercetin supplement. In a test tube, quercetin can slow down a liver enzyme called CYP2C8. The theory is that blocking this enzyme could let some drugs build up a bit, which might mean slightly more side effects. For Epclusa, that could look like headache, tiredness, or nausea.

The honest part: this is a theoretical concern based only on lab data, and a human study with a similar drug showed no real effect. So I wouldn't lose sleep over it, especially with food amounts of quercetin. Still, because clearing this virus matters, run any high-dose quercetin supplement past your pharmacist or doctor first.

What Is Sofosbuvir and Velpatasvir Used For?

Brand name: Epclusa

Sofosbuvir and velpatasvir treats chronic hepatitis C virus (HCV) infection caused by genotypes 1, 2, 3, 4, 5, or 6 — covering all the major strains. It is approved for adults and children 3 years of age and older, whether or not they have cirrhosis (liver scarring).

It is used on its own in patients without cirrhosis or with compensated cirrhosis (where the liver is scarred but still functioning). In patients with decompensated cirrhosis (more advanced liver disease where the liver is no longer coping), it is used in combination with another medicine called ribavirin. Epclusa is a brand that is also available as oral pellets for younger children.
