Why this applies to Sulfamethoxazole
Sulfamethoxazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses a liver enzyme called CYP2C9 to help clear sulfamethoxazole, and quercetin can slow that enzyme down. In theory, that means sulfamethoxazole could hang around a bit longer and build up slightly, which might nudge up the chance of side effects like nausea, stomach upset, headache, or skin rash. This is an active drug (not a prodrug), so blocking the enzyme tends to increase its effect, not weaken it.
Honestly, the evidence here is thin and mostly based on lab work and studies of other drugs, so it's more of a caution than a red flag. Food amounts of quercetin are very unlikely to matter. If you're taking high-dose quercetin supplements, just give your pharmacist or doctor a heads-up before combining them.
