Sulfamethoxazole, Trimethoprim falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the simple version. The sulfamethoxazole half of Bactrim is partly cleared by a liver enzyme called CYP2C9. Quercetin supplements can slow that enzyme down, so in theory the drug could clear a little more slowly and build up in your system. That might mean a slightly higher chance of typical Bactrim side effects, like nausea, an upset stomach, a skin rash, or sun sensitivity.

I want to be honest about how strong this is. The evidence comes mostly from lab work and studies on other CYP2C9 drugs, not Bactrim itself, so it's more theoretical than proven. Food amounts of quercetin are very unlikely to matter. If you're taking high-dose quercetin supplements, just run it by your pharmacist or doctor first.
