ANTIHYPERTENSIVE DRUGS

Treprostinil falls under ANTIHYPERTENSIVE DRUGS — the group Reishi Mushroom interacts with.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Treprostinil (Orenitram) already lowers blood pressure as part of how it widens blood vessels in your lungs. Reishi mushroom has shown its own mild blood-pressure-lowering effect in some studies, so taking the two together could, in theory, push your pressure down a bit more than expected. That might leave you feeling dizzy, lightheaded, flushed, or faint, especially when you stand up quickly.
I want to be honest about the evidence, though. This concern comes mainly from animal and lab research, plus mixed human results, so it's more theoretical than proven. It's not a reason to panic. Just don't start reishi on your own. Check with your pharmacist or doctor first, and let them know if you notice new dizziness.

A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.

Here's the thing to keep in mind with Treprostinil (Orenitram): on top of widening blood vessels for pulmonary hypertension, it also makes platelets a little less sticky, which can raise bleeding risk on its own. Reishi mushroom may do something similar. In lab and animal studies, higher doses of reishi (around 3 grams a day) appeared to slow platelet clumping, while smaller amounts (about 1.5 grams) did not seem to.
So stacking the two together could, in theory, add up to more bruising, nosebleeds, or harder-to-stop bleeding. I'll be honest though: this is theoretical and based on weak evidence, not real-world cases. Still, please run it by your pharmacist or doctor before starting reishi, especially at concentrated supplement doses.

About Treprostinil
What Is Treprostinil Used For?
Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.
Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
