Reishi Mushroom and Vorapaxar

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Vorapaxar

Vorapaxar falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Reishi Mushroom interacts with.

Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind: Vorapaxar (Zontivity) is already a blood thinner that stops platelets from clumping together, and it's often taken alongside aspirin and clopidogrel. Reishi mushroom, in higher amounts, may have a mild blood-thinning effect of its own. Stack those together and you could, in theory, tip the scales toward easier bleeding, things like bruising more readily, nosebleeds, bleeding gums, or blood that takes longer to stop.

I want to be honest about the evidence, though. This concern comes mostly from lab studies, and one human study found that about 1.5 grams a day didn't affect platelets, while 3 grams a day did. So culinary amounts likely matter little, but concentrated doses could. Please check with your pharmacist or doctor before adding reishi.

From our pharmacist’s overview on the Vorapaxar page

“Vorapaxar (Zontivity) keeps platelets from clumping after a heart attack or clogged leg arteries, so it raises bleeding risk by design, and adding supplements calls for real caution. Of the 282 ingredients on file, 202 can thin the blood further, stacking their antiplatelet effects on the drug's.”

Reishi Mushroom
  • Immune support
  • Fatigue and general well-being
  • Stress and sleep
  • Inflammation
  • Heart and cholesterol health

Reishi is a traditional Asian mushroom widely used to support the immune system and overall wellness. Human evidence for most of its claimed benefits is limited or low-quality, so it should not replace proven treatments. It is generally well tolerated for short-term use, but check with your pharmacist or doctor before starting, especially if you are pregnant, nursing, or take other medicines.
