Diltiazem, Enalapril falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Resveratrol interacts with.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Teczem combines two blood pressure medicines: diltiazem and enalapril. The diltiazem part is the piece that matters here, since your body clears it using a liver enzyme called CYP3A4. In test-tube studies, resveratrol can slow that enzyme down a bit. In theory, that could let diltiazem build up slightly, which might add to its effects like lower blood pressure, a slower heartbeat, dizziness, or swollen ankles.
Here's the reassuring part: this is based on lab data only, not real patients. In one human study, even a large 3000 mg daily dose of resveratrol didn't require any medication changes. So for most people this is unlikely to cause trouble. Still, check with your pharmacist or doctor before adding resveratrol, and keep an eye on your blood pressure.
