Cefixime falls under NEPHROTOXIC DRUGS — the group Rhubarb interacts with.

Theoretically, long-term use of anthraquinones from rhubarb might increase the risk of nephrotoxicity when used with nephrotoxic drugs.
The anthraquinone constituents of rhubarb have been shown to induce nephrotoxicity in animal research. Additionally, in a case report, a 23-year old female presented with kidney failure after taking 6 tablets of a proprietary slimming agent (found to contain the anthraquinones emodin and aloe-emodin from rhubarb) daily for 6 weeks and then adding diclofenac 25 mg 4 times daily for 2 days. The authors postulate that the anthraquinone constituents of rhubarb contributed to the renal dysfunction, and the addition of diclofenac, a nephrotoxic drug, led to renal failure. Until more is known, advise patients to avoid taking rhubarb if they are taking other potentially nephrotoxic drugs.

Here's the simple version: rhubarb root contains plant compounds called anthraquinones, and in animal studies these have been hard on the kidneys. Cefixime (Suprax) is a cephalosporin antibiotic that your kidneys help clear out, and like many antibiotics it can occasionally put some stress on them too. The worry is that pairing the two over time might add up and raise the chance of kidney trouble, things like changes in how much you urinate or swelling.

I want to be honest about how thin this evidence is. It's theoretical, based mostly on lab and animal work plus one case report involving a different drug. Eating rhubarb stalks in a pie won't matter here. Still, while you're on cefixime, I'd skip concentrated rhubarb root supplements and check with your pharmacist or doctor first.

About Cefixime

What Is Cefixime Used For?

Brand name: Suprax

Cefixime treats several bacterial infections in adults and children six months and older. These include uncomplicated urinary tract infections (UTIs), otitis media (ear infections), pharyngitis and tonsillitis (strep throat and tonsil infections), acute exacerbations of chronic bronchitis (sudden worsening of ongoing bronchitis), and uncomplicated gonorrhea (cervical or urethral).

Cefixime tablets are indicated only for adults and children weighing more than 45 kg or older than 12 years, and tablets are not approved for otitis media — ear infections must be treated with the oral suspension or, where available, chewable tablets. Cefixime oral suspension and capsules are approved for the same infections across the full pediatric age range (six months and older).
