Macitentan, Tadalafil falls under ANTIHYPERTENSIVE DRUGS — the group Soy interacts with.

Theoretically soy protein may have additive effects with antihypertensive drugs and increase the risk of hypotension.
Although some contradictory research exists, most clinical evidence suggests that consuming soy protein modestly reduces systolic and diastolic blood pressure in individuals with prehypertension or hypertension.

Opsynvi combines two blood pressure lowering medicines: macitentan (which relaxes blood vessels in the lungs) and tadalafil (the same drug in Cialis). Both can nudge your blood pressure down. Here's the wrinkle with soy: research suggests soy protein can modestly lower blood pressure on its own. So stacking regular soy intake on top of Opsynvi could add up and push your pressure a little lower than expected.

If that happens, you might feel dizzy, lightheaded, or faint, especially when standing up quickly. The good news is this effect is usually mild, and everyday soy foods like tofu or edamame are far less likely to matter than concentrated soy protein powders or isoflavone supplements. If you use those regularly, mention it to your pharmacist or doctor so they can keep an eye on your readings.

Macitentan, Tadalafil also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Soy might modestly induce CYP2C9 enzymes. However, this effect does not seem to be clinically significant.
In vitro research suggests that an unhydrolyzed soy extract might induce CYP2C9. However, the significance of this interaction is likely minimal. In healthy females taking a specific extract of soy (Genistein Soy Complex, Source Naturals), blood levels of losartan, a CYP2C9 substrate, were not significantly affected.

Good question about pairing soy with Opsynvi, which combines macitentan and tadalafil for pulmonary arterial hypertension. The concern on file is that soy might speed up an enzyme called CYP2C9. In theory, if that enzyme were revved up, it could clear a medication a little faster and weaken it. But here's the reassuring part: this is based only on lab (test tube) research, and when soy was actually tested in people, blood levels of a similar CYP2C9 drug didn't really change.

So for practical purposes, eating soy foods like tofu or edamame is very unlikely to affect how well your Opsynvi works. If you're considering a concentrated soy isoflavone supplement, just run it by your pharmacist or doctor first to be safe.
